Separate Policies and Procedures.
Material and Information reside in Quality Foundation:
Locally established Clinical Pathways;
Minutes;
Reports;
Analysis.
Distilled information only to reside in Physician’s file:
Recommendations on credentialing, education, or disciplinary action are only recorded in a distilled form not to include reports or analysis leading to recommendations.
The Quality Foundation has no statutory obligation to report information to anyone but its client and in a form acceptable to the client.
